Vision Benefits

The EyeMed Vision Care website allows you to: 

• Verify eligibility

• Obtain a list of participating providers

• Download an out-of-network claim form

• Download a Student Status Verification Form

• Check the status of an out-of-network claim

• Recommend a provider for participation on the EyeMed panel

You must access the EyeMed web site from the Department of Civil Service web site at www.cs.state.ny.us. Click on Benefit Programs, then NYSHIP Online, Other Benefits, then Vision Benefits.  

 Print a handy brochure with tips to help you maximize your benefits the next time you're scheduled to have a vision exam. This file can only be viewed with Adobe Acrobat Reader which can be downloaded for free at Adobe.
