To find a participating dentist follow these steps:
Go to the Civil Service web site (www.cs.state.ny.us) Click on Benefit Programs, then NYSHIP Online, Other Benefits, then Dental. You can review benefits, check the non-participating provider reimbursement schedule, and download a claim form from the Civil Service site. There is also a link to the GHI web site, which has an online provider directory and allows you to to review claims submitted.
